Puerto Rico's Extreme Trans Healthcare Ban: The Harshest in the US

Puerto Rico just passed the most extreme trans healthcare ban in the entire United States or any territory. Comment below if this makes you angry.

On July 17th, 2025, Puerto Rico banned gender-affirming care for anyone under 21. Not 18. Twenty-one years old.

Let that sink in. In Puerto Rico, you can vote at 18. You can join the military at 18. You can get married at 18. You can drink alcohol at 18. But you cannot access healthcare until you turn 21.

And if a doctor tries to help you? Fifteen years in prison. Fifty thousand dollars in fines. Permanent loss of their medical license. Forever.

This isn’t just about new patients. Kids who are already on hormone therapy have to stop. Forced medical detransition by law.

And here’s the cruelest part. Puerto Rico is an island. You can’t just drive to the next state like kids on the mainland can. You’re trapped there until you turn 21.

The governor ignored her own Health Secretary who opposed this ban. She signed it anyway.
